Tracing the Microbiome Shift in South Asian Cities

As India faces a ballooning health crisis with 89.8 million diabetic adults as of 2024, new research suggests the culprit lies not just in genetics but in the gut. A three-year investigation published in the journal Gut Microbes has established the first reference database for the South Asian gastrointestinal microbiome. By sampling 575 individuals from 10 distinct communities, an international team led by Niraj Rai of the Birbal Sahni Institute of Palaeosciences and Maanasa Raghavan of the University of Chicago identified how the transition from traditional to processed diets reshapes internal biology.

The study utilized a unique demographic loophole: India’s co-existing rural and urban populations who share the same ancestry but live in vastly different food environments. Researchers paired forest-dwelling or early-agriculturalist groups with genetic relatives who had migrated to urban centers. This experimental design allowed scientists to isolate environmental impacts from genetic ones. While gene sequencing confirmed shared heritage, the microbial profiles diverged sharply once subjects began consuming processed foods like soft drinks, pizza, and biscuits.

The Rise of Megamonas and the Loss of Protective Bacteria

The research identified Megamonas as a primary marker of urbanization. This bacterium was significantly more prevalent in urban dwellers and showed a direct correlation with elevated blood glucose levels. Globally associated with hypertension and obesity, Megamonas appears to thrive on the lipid-rich, processed diets found in cities. Conversely, the urbanization process appears to strip away beneficial microbes such as Holdemanella.

  • Holdemanella: Linked to glucose tolerance and fiber intake; largely disappearing in urban populations except in regions like Kerala, where traditional diets persist.
  • Megamonas: Strongly associated with higher blood sugar; thrives on the "pizza, chowmein, and coke" diet typical of city life.
  • Lactose Processing: Data suggests that specific gut bacteria allowed ancient pastoral communities to digest milk long before the arrival of the lactase-persistence gene from Anatolia and the steppe.

A Closing Window for Discovery

The International Diabetes Federation warns that India’s diabetic population could swell to 156.7 million by 2050. Niraj Rai argues that understanding the microbiome is as vital as studying inherited risks, comparing the bacteria to environmental stressors like smoking that trigger genetic expression. The research team is now expanding the study to include metabolomics to identify the specific molecules these bacteria produce.

Time for this research is limited. As infrastructure expands into previously isolated regions, including the Andaman and Nicobar Islands where sampling is slated for this winter, traditional "ancient gut signatures" are vanishing. Rai notes that the rapid arrival of roads and processed goods means the opportunity to study these untouched biological profiles is quickly disappearing.
