Rajasthan Launches SETU: India's First Digital Bridge for Real-Time Emergency Referrals

Eliminating 'Blind Arrivals' via Digital Integration

Rajasthan has become the first Indian state to deploy a comprehensive digital medical emergency referral framework, designed to optimize the 'golden hour' of clinical intervention. Known as Simplified Emergency Transfer and Urgent Referral (SETU), the system connects remote Primary Health Centres (PHCs) and Community Health Centres (CHCs) directly with urban tertiary hospitals. The initiative aims to end the practice of 'blind arrivals,' where specialists are unaware of a patient's condition or arrival time until the ambulance reaches the facility.

Infrastructure and Operational Mechanics

Principal Medical and Health Secretary Gayatri Rathore confirmed on August 1, 2026, that SETU operates without requiring expensive hardware. The technical framework relies on:

• Smartphones and QR Codes: Physicians at rural clinics use a dedicated app to trigger referrals.
• Closed User Group (CUG) Networks: Secure communication channels alert specialists in departments including trauma, surgery, pediatrics, and obstetrics.
• Live GPS Tracking: Control rooms monitor the ambulance’s location and estimated time of arrival in real-time.
• Data Synchronization: Digital patient histories reach specialists before the patient arrives, allowing for 'minute zero' treatment readiness.

Scalability and Early Performance Metrics

Developed and piloted at the Rabindranath Tagore Medical College in Udaipur, SETU has already processed more than 3,510 emergency cases. The program currently spans 15 districts across the state. According to Ms. Rathore, the system ensures that ICU beds and emergency staff are reserved and ready before the ambulance doors even open. To maintain quality control, the central control room conducts follow-up calls the day after a referral to gather feedback on hospital services and treatment outcomes.
