The Rise of Hyper-Endemic Transmission

India’s struggle with dengue is entering a more dangerous epidemiological period. According to a comprehensive two-year study led by the Indian Council of Medical Research’s (ICMR) Virus Research and Diagnostic Laboratory (VRDL), the country now faces hyper-endemic transmission. This state occurs when multiple virus serotypes—specifically DENV-1, DENV-2, DENV-3, and DENV-4—persist concurrently within the same population. The analysis, which examined 6,889 laboratory-confirmed samples across 25 States and Union Territories from 2023 to 2025, found that eight regions are currently hosting all four strains at once, while 15 others are battling three strains simultaneously.

Clinical Implications of Concurrent Infections

The surveillance data highlights a shift in patient outcomes. While DENV-2 remains the primary strain, accounting for over 50% of diagnosed cases, approximately 7% of patients are now infected with more than one serotype at the same time. These concurrent infections are no longer rare. Data indicates that individuals carrying multiple strains are more prone to developing:

  • Thrombocytopenia (critically low platelet counts)
  • Haemorrhagic manifestations
  • Intense joint pain and severe weakness

While recovering from one serotype grants lifelong immunity to that specific version, it only offers fleeting protection against the others. Subsequent infections with a different strain often trigger more aggressive immune responses, potentially leading to respiratory distress, organ failure, or fatal shock.

Surveillance as a Predictive Tool

Molecular tracking provides a level of foresight that standard case reporting lacks. By identifying exactly which strains dominate particular regions, health officials can anticipate changes in disease severity before hospital admissions surge. This data is vital for hospital readiness and targeted mosquito control efforts. Furthermore, the presence of multiple circulating strains complicates the development of effective vaccines, as any future immunization strategy must provide robust coverage against all four serotypes to avoid inadvertently increasing the risk of severe secondary disease.

Managing the current crisis requires eliminating standing water, utilizing repellents, and maintaining high community awareness, as no specific antiviral treatment exists for the virus once contracted.
