Data from the INSPIRE clinical trial, published in the journal Nutrients, suggests that managing gut health offers a viable strategy for treating knee osteoarthritis (OA). This condition affects hundreds of millions globally, often resulting in long-term disability. Researchers discovered that daily intake of inulin—a prebiotic fiber found in vegetables like chicory root and Jerusalem artichokes—not only decreased physical discomfort but also enhanced grip strength and reduced nervous system pain sensitivity.

Comparative Analysis of Fiber and Physical Therapy

The six-week study involved 117 adults divided into four cohorts receiving either inulin, digital physiotherapy-supported exercise (PSE), a combination of both, or a placebo. While both the fiber supplement and the exercise program reduced knee pain, the inulin group demonstrated a remarkably higher retention rate. Only 3.6% of those using the supplement withdrew from the study, contrasted with a 21% dropout rate in the physiotherapy group. Lead author Dr. Afroditi Kouraki of the University of Nottingham School of Medicine noted that the ease of incorporating a supplement into a daily routine likely contributed to this superior adherence.

The Gut-Muscle-Pain Axis

The biological mechanism behind these improvements involves the production of short-chain fatty acids like butyrate. When gut bacteria ferment inulin, they produce butyrate, which influences systemic inflammation and pain pathways. Participants taking inulin showed elevated levels of both butyrate and glucagon-like peptide-1 (GLP-1), a hormone tied to muscle health and pain regulation. Senior author Professor Ana Valdes highlighted the correlation between higher GLP-1 levels and improved grip strength, suggesting the existence of a "gut-muscle-pain axis" that impacts physical resilience during aging.
