Diagnostic Research Targets Biomarkers to Forecast Severe Dengue Trajectories
Scientists at the Institute of Advanced Virology are developing a prognostic blood protein test to identify high-risk dengue cases. This diagnostic tool aims to predict hemorrhagic shock before clinical warning signs manifest.
Advancing Early Detection in Hyperendemic Zones
Clinicians currently face a significant hurdle in treating dengue: the inability to anticipate which patients with mild symptoms will deteriorate into life-threatening hemorrhagic shock. While the World Health Organization outlines several red flags, including mucosal bleeding, abdominal pain, and lethargy, these indicators often surface only after internal pathologies are well underway. In response, the Institute of Advanced Virology (IAV) in Kerala has launched a large-scale study involving 2,000 patients to identify biological indicators capable of forecasting disease severity during the early febrile stage.
Pilot Results Reveal Precision in Prediction
A recently completed pilot study of 150 patients demonstrated that specific blood proteins remain stable during initial admission but spike dramatically during the fever-break stage. This surge is significantly more pronounced in victims who eventually develop severe complications compared to those with mild infections. Lead researcher Agiesh Kumar B. noted that by integrating these endothelial markers with established clinical signals, a computational model could predict severe outcomes with nearly 100% accuracy.
Strategic Partnerships and Scale
The IAV has established a formal Memorandum of Understanding with the Government Medical College in Thiruvananthapuram, Believers’ Church, and Pushpagiri medical colleges. This collaboration facilitates extensive sample screening and long-term clinical monitoring. Key project components include:
- Focus on Endothelial Integrity: Researchers are targeting markers released by activated endothelial cells, which indicate the plasma leakage characteristic of severe dengue.
- Comprehensive Screening: The initial list of 25 potential biomarkers has been distilled to a refined panel of 8 to 10 candidates for intensive scrutiny.
- Global Technical Support: The National University of Singapore is providing computational modeling expertise to enhance the predictive reliability of the findings.
Optimizing Healthcare Resources
In regions like Kerala, where seasonal outbreaks frequently overwhelm hospital capacity, the necessity for a bedside diagnostic tool is high. An affordable prognostic assay would allow medical staff to prioritize inpatient care for those at genuine risk while managing more stable patients through outpatient protocols. The IAV team, supported by Director Sreekumar E. and scientist Abhinand C.S., intends to develop an indigenous point-of-care device, potentially with assistance from the Department of Science and Technology, to translate these laboratory findings into a practical clinical tool.
